Types of Applications

Any qualified applicant may file any one (1) of the following eight (8) types of Application before the EO:

  1. New registration;
  2. Transfer from another district, city or municipality;
  3. Transfer within the same district, city or municipality due to change of address;
  4. Change of name;
  5. Correction of entries;
  6. Inclusion in the Book of Voters;
  7. Reinstatement to the Voters List; and
  8. Reactivation.

  1. New registration:

    CEF 1A

    This Application is for any qualified applicant who has never filed an Application for Registration all his life, such as someone who just turned 18 years old, or whose name is not included in any Book of Registered Voters in the country.

    The Application Form for Registration, which is also known as CEF 1-A, may be obtained, free-of-charge from the EO’s office or may be downloaded from this website. Please take note that this application has to be accomplished in three (3) copies. The copy for the EO has to be printed back-to-back.

  2. Transfer from another district, city or municipality:

    CEF 1B

    This Application is for any registered voter who has transferred residence from another HUC district, from another city or from another municipality at least six (6) months before Election Day.

    The Application Form for Transfer, which is available on green paper, may also be obtained free-of-charge from the EO’s office. However, is NOT available for download from this Website for registration use, but is only shown here for information purposes so that you may know how it looks like. Even if you are able to download a copy from this Website, you will not be allowed to use it in filing your application.

  3. Transfer within the same district, city or municipality due to change of address:

    CEF 1B

    This Application is for any registered voter who has changed his (or her) address or has transferred residence within the same district, within the same city or within the same municipality.

    The Application Form for Transfer is the same for transferring from another or within the city or municipality. It is NOT available for download from this Website for registration use, but is only shown here for information purposes so that you may know how it looks like.   4. Change of name:

    CEF 1D

    This Application is for any registered voter whose name has changed by virtue of marriage, or by court order or order by the Civil Registrar or Consul General.

    The Application Form for Change of Name, which is available on blue paper, may also be obtained free-of-charge from the EO’s office. However, is NOT available for download from this Website for registration use, but is only shown here for information purposes so that you may know how it looks like. Even if you are able to download a copy from this Website, you will not be allowed to use it in filing your application.

  5. Correction of entries:

    CEF 1D

    This Application is for any registered voter whose registration record in the Precinct Book of Voters or Certified List of Voters contains erroneous entries, including wrong or misspelled name, birth date, birth place or typographical errors.

    The Application Form for Correction of Entries is the same as the form for Change of Name. As with all the other Application Forms, aside from that for New Registration, it is NOT available for download from this Website for registration use, but is only shown here for information purposes so that you may know how it looks like. Even if you are able download a copy from this Website, you will not be allowed to use it in filing your application.

  6. Inclusion in the Book of Voters:

    CEF 1D

    This Application is for any registered voter whose name has not been included or has been omitted in the Precinct Book of Voters.

    The Application Form for Inclusion in the Book of Voters is in the lower bottom half of the Application for Change of Name. As with all the other Application Forms, aside from that for New Registration, it is NOT available for download from this Website for registration use, but is only shown here for information purposes so that you may know how it looks like. Even if you are able to download a copy from this Website, you will not be allowed to use it in filing your application.

  7. Reinstatement to the Voters List:

    CEF 1D

    This Application is for any registered voter whose name has not been included or has been omitted in the Precinct Certified List of Voters.

    The Application Form for Reinstatement in the List of Voters is the same as that for Inclusion in the Book of Voters. As with all the other Application Forms, aside from that for New Registration, it is NOT available for download from this Website for registration use, but is only shown here for information purposes so that you may know how it looks like. Even if you are able to download a copy from this Website, you will not be allowed to use it in filing your application.

  8. Reactivation:

    CEF 1C

    This Application is for any registered voter whose registration record has been deactivated based on grounds provided under Section 27 of Republic Act No. 8189. The most common reason for the deactivation of a registration record is the failure of a registered voter to vote in two (2) consecutive regular elections, such as in both the 10 May 2010 National and Local Elections and the 25 October 2010 Barangay Elections.

    The Application Form for Reactivation, which is available on yellow paper, may also be obtained free-of-charge from the EO’s office.
