In connection with the 01 December 2009 Submission of Application for Eligibility and to Bid and Opening of Bids anent the subject matter, the public and the prospective bidders are hereby informed of the following matters:

1. To delete the letters "HP" each written and provided under the Articles/Service Specification of Items 14, 15, 16 and 17, respectively, of the Bidder's Tender.


2. For purposes of this bidding, the words "per item" means the same thing with the words "per lot".


3. To require prospective bidder to present proof of its single largest contract in a "per item" basis, provided that the contract to be presented is the same or closely related to the goods that the supplier wanted to bid. This holds true in posting a bid bond or bid security, it shall be made also in a "per item" basis.


4. To require all prospective bidders in the bid opening to bring with them sample of the goods they wanted to bid for purposes of technical evaluation.
