In connection with the 17 December 2009 Submission of Application for Eligibility and to Bid and Opening of Bids anent the subject matter, the public and the prospective bidders are hereby informed of the following matters:

1. Each document to be submitted by the bidder in satisfaction of the bidding requirements must be original copy or if this is not possible, certified true copy, provided there is an affidavit stating that the same are true duplicate of the original.


2. The BAC only allows surety bond that was issued by a surety or insurance company duly certified by the Insurance Commission.


3. The awardee is required to deliver the goods subject of the contract not later than 15 days upon receipt of the purchase order.


4. The specifications mentioned in Lot Nos. 1 and 2 of the Technical Specification of the Bidding Documents are amended as follows:

5. The winning bidder is advised to get sample of Lot Nos.1 and 2 from the Committee on Printing of Non-Accountable Forms and the blueprint of the same shall be approve for printing by the Chairman of the said Committee.
