Pursuant to BAC Resolution No. 98-2010 dated 29 March 2010 and approved by the Commission En Banc on 29 March 2010, you are hereby notified that the Commission AWARDS to your firm the contract for the supply and delivery of the following:

| Sealed dripper type, Plastic bottle of 15 ml. without side and “side-bottom” seams to ensure that bottle container is one-piece and that no adulteration can occur before the issuance to BEI, contains at least 7% silver nitrate, with commassie blue or blue dye, not more than 1.75% of other additives and solvents, the following solvent: Ethyl Alcohol, benzene, gasoline, soap, solvent, oxalic acid, 95% bleaching solvent, acetone, lacquer thinner, brake fluid, ascorbic acid, water and kerosene will not remove the stain from nail, cuticle and the first joint of the finger (second distal phalanx), each plastic bottle will be appropriately labeled through direct printing on the bottle, two (2) 15 ml. plastic bottle will come in a plastic pouch with the printing “COMELEC INDELIBLE STAIN INK TWO (2) 15 ML BOTTLES” |

Within five (5) days from receipt of this Notice of Award, your firm shall post with the Commission a performance security in the amount equivalent to five percent (5%) of the contract price in the form of either cash or manager’s/cashier’s check payable to Commission on Elections. Your failure to post the requisite performance security shall be ground for the annulment of this award.
