2. To change the word “Lot” written under No.2 of the Invitation to Bid to “Lots”.


3. To change the words “Fixed Length Seals” written under the Description, Lot 4, No.5, Invitation to Bid, to “Security Seals”.


4. To change the words “one key per padlock” written in Lot 1, ITB Clause 1.2 of the Bid Data Sheet to “THREE KEYS per padlock”.


5. To add the phrase “SHELF LIFE/EXPIRATION SHOULD BE GOOD FOR THREE (3) MONTHS FROM THE DATE OF DELIVERY TO COMELEC.” after “15 ML BOTTLES” written under Lot 2, ITB Clause 1.2 of the Bid Data Sheet.

8. To insert the numbers “5 and 6” to the space indicated for Lot No., adjacent to the words “Self-Locking Serially Numbered SECURITY Seals” written immediately below Lot No.4, Total ABC Per Lot, Clause 1.2, Bid Data Sheet.


9. To change the words “Fixed Length Seals” written under Lots 5 and 6, Clause 1.2, Bid Data Sheet to “SECURTY Seals”.


10. To change the words “one key per padlock” written under the Supply and Delivery, Lot 1, Clause 1.2, Bid Data Sheet, to “THREE KEYS per padlock”.

15. To change the words “one key per padlock” written under the Description, Lot No.4, Technical Specifications, to “THREE KEYS per padlock”.


16. To add the phrase “SHELF LIFE/EXPIRATION SHOULD BE GOOD FOR THREE (3) MONTHS FROM THE DATE OF DELIVERY TO COMELEC.” immediately after the description “15 ML BOTTLES” stated under the Specification, Lot No.1, Schedule of Requirements.


17. To change the words “Made of yellow PP plastic” written under the Specification, Lot No.5, Technical Specifications, to “made of yellow PA66 (POLYAMIDE).”


18. To change the words “Adjustable within the belt as desired” written under the Specification, Lot No.5, Technical Specifications, to “WITH adjustable belt.”


19. To add the figures “120MM” immediately after the word “Tensile strength:F>400N” written under the Specification, Lot No.5, Technical Specifications.


20. To change the words “Tensile strength : >500N” written under the Specification, Lot No.6, Technical Specifications, to “Tensile strength: >5000N”


21. To add as second sentence of 13.1 of the Bid Data Sheet the phrase “Bidders are required to submit a “Certification” from the Department of Science and Technology (DOST) that the indelible stain ink to be supplied shall have a shell life/expiration good for three (3) months from the date of delivery to COMELEC.”
